This Complaint and Feedback Form has been developed to facilitate external communication and provide a platform where feedback and complaints can be lodged. This template provides a table where details of the complaint or feedback can be entered including project or worksite details, type and description of complaint / feedback, details of external party, advised corrective, preventative or improvement actions and sign off.
